Our service will help clients to develop a wellness plan based on an individualised holistic approach. There are 3 phases to the implementation of the plan: recovery, maintenance and relapse support. Each phase is based on the needs of the client at the time with the aim to assist clients to become as independent as possible with their mental health journey.

Relapse Support Phase

Support during a relapse:

This phase provides current clients, who are experiencing a relapse in their mental health, an increase in support with the aim to prevent further deterioration. The nurse consultant will work with the client and carers to implement strategies in the wellness plan to stabilise and return to the recovery and maintenance phase or support them in engaging with the public/private mental health systems.

We aim to respond within 48 hours and review every 2/3 days as required.

NB: This service is not an acute mental health service and clients will be supported to access acute supports if needed through emergency services or their local area mental health services.

Recovery Phase

Developing and implementing a Wellness Plan:

Working with a nurse consultant to understand your mental health history and identify areas of strengths and challenges. Using this information you will work together to create a plan to guide your journey focussing on the following 3 areas: Education, understanding your mental health and holistic wellness.

Weekly/ Fortnightly sessions to develop and implement a wellness plan.

Maintenance Phase

Maintenance of wellness plan:

  • Check in sessions (as required) with clients and their carers to provide support for the ongoing implementation of the wellness plan.

  • Regular check ins with our clinical nurse consultant

  • Regular reviews of the wellness plan.

Sessions as needed and decided upon together.
